Where Do I Fill Out the Warranty Activation Form for My Tub?

You fill out the warranty activation form online at ellasbubbles.com/warranty-activation, or on the printed two-page form that ships inside your owner's manual. Either way it is signed by both the owner and the installer and returned with a copy of the sales receipt. For a description of the document itself, see What Is the Warranty Activation Form for an Ella Walk-In Tub?. This page shows where to find the form, what to have beside you and who completes each part.

The Places You Will Find the Form

There are two versions of the same form.

  • Online. Go to ellasbubbles.com/warranty-activation. The same page is linked at the bottom of every page of ellasbubbles.com, so you can reach it from the footer wherever you are on the site. No receipt number or dealer login is needed to open the form, and the online form covers all models.
  • In your owner's manual. Every Ella walk-in tub ships with the activation form in its owner's manual. Some owners know it as the warranty registration form or the installer checklist, because its second page is a list of installation checks the installer initials. If that is the page you have been looking for, it is the same document.

Whichever version you use, the form puts your Lifetime Limited Warranty on file with Ella's Bubbles, so the same details and the same two signatures are needed.

What to Have Beside You Before You Start

The form goes quickly when everything is on the table first. Gather these items:

  • Your name, contact details and the installation address. The warranty belongs to the first owner at the original installation address, so this is the address where the tub is installed.
  • The serial number. It is on the label behind the large magnetic access panel next to the door. The panel is held on by magnets, so pull it straight off, with no tools needed. A second copy is on the back wall side of the tub shell, and the serial number is also included in the shipping and tracking information sent when the tub left the warehouse. Where to find the serial number on your Ella walk-in tub shows each spot.
  • The model number, sometimes called the SKU, which appears on your proposal or invoice. It is different from the serial number: the model number is shared by every tub built in that configuration, while the serial number is unique to your tub.
  • The purchase date, and a copy of the sales receipt.
  • The installer's electrician license and insurance information.

The serial tag carries two sets of numbers. The first set is the manufacture date of the tub, and the second set is the unit's own serial number. Serial formats vary, and some begin with a letter, so copy the serial number exactly as printed.

The form also includes a purchase amount field. That figure goes into the warranty record only, and it is not printed on anything Ella sends to the customer.

Who Fills in Which Part

The form has two parts, and both carry a signature line.

  1. The owner part. This asks for the owner's contact details and the installation address. The owner signs to confirm the details.
  2. The installer part. This asks for the installing company's details and a sign-off on the installation checkpoints. The installer initials 16 items that show the tub was installed the way the warranty expects. Among them: the tub is level with all legs touching the floor, the tub is on a dedicated GFCI breaker, the thermostatic control valve was operated on both hot and cold, the locking nut is tightened on both drain stoppers, and the door seal was watertight after a 20 minute fill.

The activation is complete only once both sections are filled in and signed and a copy of the sales receipt is included. Three common situations change who does the typing:

  • Your dealer or installer submits it for you. A dealer or installer who sold or installed the tub can open the form, enter the customer as the owner and their company as the installer, and submit it. The owner's name and the installation address go on the form, never the shop address. The steps are in how dealers and installers activate the warranty in a customer's name.
  • You submit it yourself. That works too, as long as your installer's details and signature are on it.
  • You installed the tub yourself. Put your own information in the installer field and initial the checklist items you completed. If a licensed plumber or electrician made the water or electrical connections, add their name, license and insurance information alongside yours. More detail is in who signs the installer part after a self installation.

If a Detail Is Missing or the Form Will Not Go Through

A missing number does not have to stop you. When a contractor or dealer placed the order, the details may never have been handed over, and the support team can look up the model number, serial number and purchase date from the order record. Open a ticket at ellasbubbles.com/support or call (800) 480-6850 and give the name the order was placed under, the installation address and the approximate purchase date.

If the tub is fully enclosed and neither serial label can be reached, look at the shipment confirmation and tracking message from when the tub shipped, which lists the serial number for the unit.

If the online form will not accept your entry, photograph the completed pages and send them through a ticket at ellasbubbles.com/support. Include the name on your order, the installation address, a photo of the serial label, and photos or scans of the signed pages.

When to Complete It and What to Keep

Complete the form promptly after installation. If it was never done, complete it now. There is no published deadline for activation and no published clause that an unregistered tub is void, so there is no need to worry if it slipped past install day.

Afterward, keep a copy of the signed form, the sales receipt and a photo of the serial label together. Warranty parts, support requests and activation all go faster with those in hand. Hydrogen tubs also ship with their individual test results, which are worth keeping with the same paperwork.
